The Elders Diary

Born to Doctor-teacher parents, I studied in Ruia College, before getting my MS in Marine Science in 1989. I then went on to get another MS in Thyroid Hormone research from Texas A&M in 1994. I work as a Project Manager and have worked with BPOs like Zenta, Retail houses like CK and FCUK, and then with the construction house- Hirco.

I enjoy teaching- and from teaching Freshmen Biology  for three years in the US to conducting ‘Safe Riding Courses’ to Harley Davidson owners- I have decided to write to educate the people who are most affected by health and other topics- the middle aged women and men who are going to land in the health trap, and the elderly who are already there in the affliction bucket- but have no one to educate them- save the visit to the doctor. My writing endeavours to educate the elderly and to influence the middle aged millennia to age gracefully.

How to avoid Diabetes- The Elders Diary 1

How to keep Diabetes away
PART I
Over the past few months, after reading a few interesting research papers and talks with a few friends from the scientific community and from holistic health communes made me pen this learning. Having studied Hormones for my MS- the one most infamous affliction caused by any one hormone, Insulin (actually by the lack of) in my field of study, made Diabetes the easy choice for an essay.
Diabetes mellitus, commonly called ‘Diabetes,’ is the condition that occurs when the body loses its ability to control blood glucose. Uncontrolled sugar due to reduced production or lack of Insulin or increased Insulin Resistance (IR)  causes havoc with every organ in the body, and its damage is permanent. 

This article focuses on Type II Diabetes, which is what happens when young adults to elders get afflicted, and what one can do to spot its signs upto a decade earlier and prevent or delay its onset.
Why Diabetes drew my attention, is because it’s the most destructive ‘condition’ that one could have- heart disease being the first mal-effect, followed by nerve, vascular, ocular and kidney damage, many of which could occur concurrently, apart from recurrent skin infections- everything that takes a toll on the hospitals, your own finances, your overall health with the new research showing that in addition to the above, Arthritis, Alzheimers' and a few other musculo-skeletal diseases could have their genesis with Diabetes- making it possibly the most expensive affliction to treat worldwide.
Approaching the 50’mark, I am quite surprised by the number of friends, acquaintances who suffer from this malady. I try and present a few points on diet, general fitness, and lifestyle modifications that could help keep diabetes away.

For starters- lets just try a few simple tests.
1. Lie down on the floor- on your back, arms by your side- now get up using the least amount of limb support- the lesser the better. If you need to roll over get on your hands and knees to then sit up, you have required the maximum- four points. If  you are able to sit up, by just folding your knees under you- you are amongst the fittest- two point of touch- and for those using a hand to support them while rising- 3 points of contact- well- you need to work on getting up using only two points.
2. Stand straight, close your eyes, hands by your sides, extend one leg forward. With your eyes closed, rotate your ankle clockwise and anti-clock ten times. Repeat with other leg, keeping eyes closed.
You will be surprised to see how many need three to four points of contact to get up and how many lose their balance when completing ten ankle rotations.
3. A quick walk test - normal people can walk a distance of 400–700 meters in 6 minutes. A 6-minute walking distance of less than 300 meter is simple and useful predictor of poor physical health, and often a very big warning towards a cardiac issue as well.

The brief logic to this- these tests are kind of home check to see  your core muscles, and your sense of balance and a quick check of your hearts condition. It’s the fastest check to let you know where you stand on a general physical ‘shape.’ There are hundreds of other exercises and ways to test for this- but these two can be done even while reading this article. Also, the article is to be treated as a ready reckoner, and by no means an alternate to your Doctor.

Research indicates two things- the genetic bias of Type II Diabetes can be generated in a single generation, and that grandparents not having diabetes may not help you, if your parents are binging on fast food, low on exercise- the two main contributors to causing Diabetes along with genetics. So be careful of what you eat, you are also creating a possibility for your progeny to have the predisposition to diabetes.

Given in brief- these three factors- genetics, food quality and exercise (lack of)- everyone scoffs and says ‘Oh, yes. I know what needs to be done.’ But no one does anything.
 I have a brief analogy to give. Smokers, if you tell them to quit, they retort-
‘What will happen? Cancer?’
To quote a friend of my fathers, the late Dr Shree Bhandare, a very popular Ortho Surgeon-
“Lung Cancer happens to the lucky, others get emphysema or chronic bronchitis- where even death will fear to come quickly. How you will suffer is upto God, but suffer you will. Quit while you can.”
The same applies for Diabetes.

The knowledge of what diabetes can do will abound in every household or social circle, with someone or the other suffering from arthritis, further aggravating his diabetes with the ever popular prednisolone,
friends who take off their shoes and keep scratching their socks as the persistent athletes foot wont go away, the injury to the toes that wont heal, and many many such silent irritants that we will choose to ignore.
Do not ignore these signs.

What do you do?
What one does is the following- carry out three tests-
a) Insulin levels (at fasting)- to be under 5 microunits/milliliter
b) an HbA1C  to be less than 5.6
c) Post prandial  Glucose Level (two hours after lunch)-should be around <120 mg/dL, but upto a max of 140 mg/dL.
You cannot cheat on one of these tests, and remember they are for your good health not anyone elses’.

A brief interpretation of results-
a) Insulin levels of >5 will be a sign of hyperinsulinemia, an unnecessary elevated level of Insulin, even when the body was at rest. This means that you are very high probability diabetes event.
b) a HbA1c >5.6-7 means again that you are in that pre-diabetic zone, with a value >8 meaning that you would need American legalese stating ‘additional action needed,’ euphemistically meaning Doctors intervention required and possible medication too.
c) Glucose values are- Normal Fasting 70-100 mg/dL and post prandial 100-140 mg/dL. Glucose levels of >140 mg/dL are almost a sure shot indication that you are a borderline diabetic and your medication (if any), exercise and diet control are not working in sync. Levels in the 190-200 mg/dL  range, or greater are just a indicative of a phenomenal increase in your risk levels for all complications associated with Diabetes and need urgent, if not immediate medical intervention.
You will be surprised to know that nearly 80% of Diabetes cases are detected more by accident than due to actual concern for the same. In the interest of your health ensure that all results are taken with a comprehensive view and not individually as there could be underlying metabolic disorders that could be causing these numbers.

PART II.
So what does one do- 

A. Simple basic exercise-
This is the least effort for the most effective results- Do the following every day.
1. Squats 4 sets of 20
2. Crunches- 2 sets of 20
3. Push ups 4 sets of 20 and
4. Sit-ups 2 sets of 20.
5. Plank exercises for your core- two or three reps till you total about 2 minutes of plank time.
You could take about 4 weeks to work up to this quantum by increasing the load every week. This should not take you more than 20-25 minutes and you are on your way. Encourage your child, sibling, friend to do this with you. Maintain a slow and steady rhythm. Once you are able to do this daily, you should replicate it in the evening before dinner as well. IF you are on any medication, consult your Doctor before starting on any exercise routine.
Any more exercise is a bonus. Swimming, walking, running, jogging- any.

These kind of exercises causes micro-tears in muscle fibers have two beneficial effects to the body- the need for ATP to be generated for energy to repair is the most easy to quantify and for those knowing that the glycolytic pathway and its associated Krebs Cycle keeps utilizing the Glucose in the body. This continual use of Glucose then reduces the stress on the Pancreatic beta cells to produce Insulin which would be needed to convert the surplus Glucose to fat. Apart from this, a little known effect seen almost within 48-72 hours of beginning exercise is the jump in the production of the hormone Follistatin- a glycosylated protein hormone, that plays an important role in muscle mass increase.

B. Simple lifestyle changes-
Park your car in the farthest parking lot, climb the last 5 floors to your house/office, descend the first five floors, keep a chart of stretching exercises that you can indulge in at your desk, cubicle- and do that every so often to keep the muscles in use. The increased activity helps your motor function, keeps your senses alert- sight, smell, balance, hearing and keeps your cognitive function in a learning mode- just like playing chess or solving crosswords and puzzles are believed to delay age-related dementia.

C. Starve.
Jokes apart.  This is serious work. Skip a meal a day, replace it with a fruit, do two days of the week on a 500-700 Calorie/day diet- like the 5:2 diet advises, eat before 7 or 8 in the evening to maximize starvation time. It may sound difficult, but salads, lentil soups and fruits will keep you in that range. The holistic believe that every religion from Islam to Hinduism preaches this- dieting and starvation in one form or the other. Few follow this advise of ‘fasting’ in other words- so I try and simplify this- by asking you to cut down on the meals in a little more ‘easy’ manner. Science is working on huge topic called ‘calorie restriction’ in primates and humans and Google will be glad to give you additional reading on how oxidative stress and chronic inflammation are reduced.
This basically means- that you proportion your food, across the week, and though binging has its mal-effects, and if you cannot avoid the ‘weekend whisky warrior’ or  ‘lounge lizard’ phenomena- then on  Sunday- just eat fruit all day long, and give your system literal ‘rest.’ A proper menu is required to ensure that you do not starve yourself off essential nutrients and create a whole other problem.

D. Remember.
One of the simplest ways to get to a better lifestyle is to start remembering what is it that you did, like climb the stairs when you came home, when was the last time you downed  pegs of a single malt, ro simpler things like, are you purple in the face when you are done tying your shoe laces. Sitting in your sofa, sagging with old age, is it a teeth gritting effort to propel yourself out of the bucket of softness that your backside is stuck in? Three pages into a novel at night, as you asleep that you cannot remember the last word you read- or if the page number was 253 or 337. These simple day-to-day activities that will serve as a barometer for your own fitness. Along with a reduction of a ‘point of contact’ as mentioned in the start of this article, you would be well on your way to avoiding diabetes, or if not- delaying it for a fruitful life for atleast another decade, free of worry.

In addition to this, as if the damage by Diabetes itself was not enough- Diabetes is now believed to be among the precursors for two diseases- Alzheimers and Prostate Gland Cancer due to its effects on levels of IGF-1 and Estradiol. There is a lot of technical data and stuff on this topic, but I have tried to keep it simple.
You don’t want to get old, and not remember how to pee.
And if you remember, not be able to.

 Stand up and exercise.

NOTE
Do not ever self diagnose your ailments, the numbers here are to prompt you to take a Doctor opinion faster than you usually would.
As regards, ayurvedic or home remedies, you could continue on them as in the least, they will have a palliative effect. These will not replace Allopathic medication.
Do not ever self medicate and never stop any allopathic medication based on anything you read. Always consult your Doctor. If in doubt, get a second opinion.

Screams at Death Time

Strange things happen in combat.
Many years ago, an ambush was laid across a dry mountain stream and there was confirmed intel that a group of 6 heavily armed militants were to cross into India through the Bhimber Galli area. These same militants, the Intel report stated were trying to cross over for the last month from different points but were being thwarted. One confirmed attempt was three weeks ago at the KG area, when the Pakistani Army pounded the Indian defences from about 1500hrs till 2100Hrs- but a freak chance pick up by the NVD monitor ensured that the 'crossing party' were denied their entry into India. The report also said they would make their second attempt at BG, where they had tried to cross just ten days earlier. Maybe the Pakistanis were playing smart, and not using their ammunition to alert the Indians this time- well time would tell.
At about 0200Hrs, a lone militant was noticed by the scout, coming walking as opposed to the 6 we were expecting.... in about 15 mins of pin drop silence the man walked through the ambush continuing east bound in the general directions of the safer plains of the Poonch River.
Our stopper, the guy who closes the ambush, in an eager show- of glee and bravery, pounced on him and after him followed about 5 others of the ambush party. The militant, taken by surprise and weighed down by his rations, ammunition, AK, and his rug for protection from the cold- was easily over powered. A sound thrashing- to put it mildly- followed and a badly beaten and bruised militant, Ahmed, was taken into custody. As per the unwritten protocol, a signal was sent to the Bn HQ- that a militant was captured in a fire fight and he was grievously injured in the exchange of fire. he is extremely aggressive and with great difficulty we have managed to restrain him. Due to bleeding, it seems unlikely that he will survive the long walk back to the Unit Lines.
With the Unit duly informed, and his clothes stripped off- he was literally frog marched to the Unit.
Five days of sustained interrogation later, it was ascertained that this was the group that was trying to cross over about ten days earlier, when their attempt was thwarted by the Unit along the LoC, but at the loss of a Unit JCO. Once that news got out, the men of the Unit was very very angry and the air was thick with revenge and lynching seemed to be the flavour of the day. The troops were getting more and more agitated as time was going by and the risk that someone being on Guard Duty  would shoot him was high. was decided to do away with him- and a young Captain was tasked to just take him away to the river and finish it off. 'Good riddance to bad rubbish' as I would remember his CO saying.
As the Captain and members of his Ghatak Platoon followed him, Ahmed realized that they were going to kill him as the soldiers started to squabble and argue about who would shoot him.
Strangely, the Units junior most officer, a young Lt, fresh out of IMA, an AEC Officer on deputation to the Sikh Unit- voiced his quiet opinion. 'Sir, after 4 months I will return to my Arm, I will never get this chance to kill a militant, Sir, please let me shoot him'
By the time the 6 men reached the stream, just 2km behind their Unit lines, they ordered a wailing and crying Ahmed to march into the water- knowing his end was high- he begged for mercy and swore on the Quran, wailing and crying hysterically, that he would never ever do this again. Immune to such stories, the Captain raised his AK- but was beaten to the draw by the young Lt, who said, in a high pitched shout- the shout sounded frantic and in a pitch more desperate than the pleadings of the soon to die, Ahmed- 'Sir please, Sirrrrrr' and managed to get his AK up and going- the first burst into the upper torso- two rounds to the upper torso- one to the nipple, one just under the clavicle, with one to the right cheekbone, while the other 25 rounds went, as wayward as the screams from the Lt's mouth.
The Captain cursed- and then calmly told the Lt 'You fucker, now go in the water and undo his wrists and ankles before blood coagulates there.'

Such is death.

Riding in the wet- Monsoon riding In India

The first showers have hit the city and here are a few points for all of us to keep in mind for a wonderful riding season.
1. Invest in good gear- there is no substitute for good riding wear. A jacket with adequate padding at the shoulders and elbow and preferably on the back as well. Inner body armor like the kind made by Icon or Dainese would definitely be an added plus. Knee guards and above-ankle shoes, should be almost mandatory. Jackets come with detachable inner waterproof liners, make sure you carry them, the HD Sling Bag can do just that. If you examine good gloves, you will find they have a 'squeegee' kind of ridge on the thumbs that works wonderfully to clear muck and grit on the windshield.
Riding while wet and cold is one of the surest ways to fall sick- therefore it is imperative that you are adequately geared to ride comfortably.
2. Clean your gear- send your jackets and body wear for wash, along with your helmets inner liners. Look up the cleaning instructions for your gloves and similarly for your shoes. In case of an emergency clean gear makes it easier to remove and not to mention healthier for your own selves.
3. Mumbai has been hit by the first rains this morning- RAINBOW PATCHES- on the road are what you should look out for and study them on your way to and from work… The initial rains will eke out the sludge left behind by vehicles and all the oil and grease accumulated from last monsoon will surface- making the first week of riding in Mumbai a very tricky affair. The emergence of bubbles on the tire marks left by vehicles ahead of you is also another sure shot indicator of the surfactants and detergents added to fuels and indicating a very high 'slippery' quotient to the roads.
This not only will happen in the first week of monsoons, but also to a smaller degree in the first 10-15 minutes of every downpour through the season, so be careful- pull over and stop, get your rain gear on, and let the rains wash off the crud. Ride again.
4. Apart from rainbow patches, out government uses a very dangerous plastic emulsion paint with reflective capabilities to warm you of junctions, bridge ramps and speed breakers (Some of them atleast)- this plastic emulsion paint has a very poor coefficient of friction and will result in a slip or skid when you need traction the most- to slow down or stop. Please ride aware of these stripes in the city and more so on the highways.
5. Everytime you stop your bike and dismount- you should keep a cloth handy to clean your headlight, tail light and indicators. You will be surprised how the road spray hits the hot headlight and almost dries leaving a layer of dried mud on the glass. Even if you ride 5 mins in the rain, or on a wet road and dismount, please clean your lights.
6. When you are riding, and you see rainbow patches or small bubbles in tire marks, please very gently carry out a traction test. Tap both your brakes simultaneously and see how the bike reacts at the slowest of speeds. This will allow you to know the condition of the road as well as your ability to control the bike with the minor shivers that you may experience- better to know this in advance than to realize that the shiver i going to result in a complete loss of control.
7. While riding, remember to 'chase a taillight-' this will simply mean that you do not ride in line with someones number plate- this allows for two things- one is a good nd clear view of the brake light of the car in front, and secondly to allow you to swerve right or left to avoid hitting the car and giving you a precious few seconds to avoid contact or damage. Given the width of the brake lights these days, you can stay in that zone and avoid the spray from the cars tires, to a large extent- and if you think the spray is hindering your progress, you are too close to the car in any case. Back off.
8. Horizontal and Linear Seperation- is what I remind all newcomers when they ride for the first time- Do not chase in a line… When riding in a group, Never chase in a line. Keep in the same lane and ride in a staggered fashion- so this allows you to see obstruction or hindrance coming up fast and also what is it that is making the rider in front brake--- and if you notice its a dog running from right to left, you will know which way the biker ahead is going to swing to avoid it… and therefore those precious seconds to ensure your personal safety and that of fellow riders. The distances of H/L separation is best decided by your earlier checks for rainbows and bubbles- and of course the traction tests.
9. In turns- we do a few things on dry roads-
a) brake before the turn
b) steady throtte while entering the turn and
c) increasing throttle while lining up for the exit
d) Shifting body weight through all of the aboe steps
while it is raining, or on wet roads, restrict yourself to one of the above at a time- it may sound difficult to do, but even if you are able to remind yourself about it, your riding will be much safer than it used to be. It encourages caution and increases your awareness of the road condition, and almost forces you to pay more attention to turns, and this results in a better and safer ride for you and those with you.
10. On Group rides, be firm with these rules, they are for everyones’ safety. Riders who don't adhere to your group riding style, have the freedom not to ride with you, and also to lead and carry on on their own, without being an hazard to other riders who wish you have good time, rather than compete against the clock.

Intolerance in India- and nation versus country?

On a thread- commenting about the AIB- and the growing intolerances here was a note I wrote- may seem abrupt, but you would get the hang of it.
"On this count- your patriotism has nothing to do with it. Corruption and Armed Forces are the only thing that keeps this country together. And it is not the job of the Armed Forces to keep us as one- the divisive forces are from within.
Pricks in the Censor Board commented that 'no script writer in his right senses should use the word Bombay in their scripts,' is this India?
The Constitution guarantees all citizens to be treated equal but as a Brahmin, I am in the minority for admission to college where my child is concerned. Is this your India?
We were never a nation before and we have not a single string of connectivity to keep us one. Our chewing gum- was the collective hatred for the British, once they left, neither the Nizam of Hyd nor the King of Kashmir wanted to be a part of this collage of people.
Only the Politicians who smelt blood and money insisted on clubbing this country and calling it a nation.
The hatred for the British is actually laughable- as they banned Sati and ensured that it was almost wiped out- and here 67 years into Independence and counting our nation has no balls to stop khap panchayats from stoning couples of different gotras...
Is this your India?
Yes, I am sure there are many good things too. Just as there are those many fools who don't know that you cannot buy land in Nagaland, yet still scream blue-murder about Article 356 in Kashmir, a right given to them in exchange of them acceding to India and more laughable (again) that it was Sardar V Patel who pushed it through the parliament- the same SVP whom the BJP hide behind these days.
India."
These are the same fools who want to take the AIB organisers to court, and they want to scream blue murder about the bad words and foul language... tomorrow, I fear those who know about my language prowess, except my dear friend Nilesh's mother who thinks KD's the kindest...- will send the moral police to my house, or a censor board notice.
Well, fuck you.

Using the Clutch

A few new riders on the Harley circuit were concerned about the clutch- on a high torque engined bike- here goes my reply.

this is one very expensive way to learn- by reading on others notes on FB.
Having warned you_
The first gear in the Harley is a kind of spring loaded shift, where the 'thunk' is engineered into the system for effect. You do not try and do anything like adjusting the clutch etc to try and eliminate this. You will only land up killing your clutch.
All other gear changes should be as smooth as 'clicks' and you should feel the gears 'notching' and move on.
Clutchless changes stress the synchromesh rings and you will eat into them- so use the clutch as provided.
On slopes, upward and down- keep the bike in gear- and under NO circumstances are you to 'free roll' or 'neutral' the bike- stay in gear and fingers OFF the clutch. If you try and bring it down on brakes, you will over heat the brakes, cause fade and crash. Simple. Unfortunately every year, we learn this about 20-24km after Amboli (west bound) when we lose a few bikes to overheated and faded brakes because of faulty riding style.
You are to use the clutch only to shift gears and you will not 'half clutch' the bike. You will eat your clutch in under 100-2500km, if you try this.
Almost- re learn to ride a high torque bike, and learn slow speed riding, as anyone can ride fast.. Learn to control the EFI system and your throttle control at speed <10kmph- that is the beauty of riding- without jerking it or half-clutching it...
I should think there will be a ride to Igatpuri on saturday morning. Do try and come along with your safety gear.'
